• Submit the completed form, attach a digital copy of your proof of purchase and select your preferred e-wallet

• By submitting this form, you confirm that you have read, understood and will respect the terms of use & PDPA.

• You will receive an SMS/email alert informing registration is processed successfully within 3 working days
